Bio Roger Jankowski

Dr Roger Jankowski is Professor and Head of the Department of Otorhinolaryngology-Head&Neck&Skullbase Surgery at the University Hospital of Nancy (France). He has described a new technique of rhinoplasty starting with a full disarticulation of the quadrangular cartilage from the vomer and perpendicular plate, followed by on demand resections of its anterior, inferior and posterior edges to adjust its size to the projection of the nasal tip when replaced on the premaxilla and in the columella pocket. This technique of septoplasty, named septoplasty by disarticulation, preserves the nasal cartilaginous dorsum. Working on the bony dorsum starts through intercartilaginous incisions, followed by elevating the skin then the preriosteum on the nasal bones, from their tip to the nasion. The periosteum under the bony dorsum is then lowered after disarticulation of the fibrocartilaginous nose from the upper arch of the piriform aperture. The bony hump can then be removed, followed by paramedian osteotomies of the remaining bony dorsum to the nasion. Bilateral low lateral osteotomies, followed by percutaneous fracture of the posterior edge of the lateral walls, allow to close the open roof and to reshape the nasal bony pyramid on the supporting olfactory fascia and in continuity with the fibrocartilaginous nasal dorsum. The olfactory fascia, according to the evo-devo origin of the nose, attaches the septolateral cartilages to the skull base. Reposition of the quadrangular cartilage on the premaxilla floor and traction sutures knotted on a buffer under the columella for a few hours secure the surgical result of a septorhinoplasty by disarticulation.
